Dr. Smith started his practice of Optometry in late 1976. We have been in this location for many years. We have convenient parking with handicap parking marked clearly. There is a portico for foul weather where patients can be dropped off and retrieved. We often valet park and retrieve your car if necessary. We ask you to fill out a questionnaire concerning your health, employment, and insurance information. Its very helpful if you have a current list of medications with you and your insurance cards. We will make every effort to prepare your insurance before your arrival with the information given to us when making your appointment.
